What It Is
SS-31 (Elamipretide) is a synthetic tetrapeptide that targets the inner mitochondrial membrane, binding to cardiolipin and improving electron transport chain efficiency. It is essentially a mitochondrial antioxidant and stabilizer.
Mechanism
In aging cells, SS-31 has been shown to restore redox balance, improve ATP production, and reduce reactive oxygen species. Animal studies are compelling: in old mice, 8 weeks of SS-31 reversed age-related mitochondrial dysfunction in muscle and heart, improving exercise tolerance and cardiac diastolic function. Long-term treatment increased healthy lifespan markers — essentially "youth-ifying" the metabolism of older animals. SS-31's effects may vary by sex: male subjects get a bigger boost in muscle endurance while females see more improvement in cognitive measures and body composition stability.

Safety & Considerations
Human trials (e.g. in heart failure) showed SS-31 is generally safe, with some patients reporting mild headaches or injection site reactions. No serious adverse events attributed to the peptide have emerged. Still, monitor blood pressure and kidney function in long courses, given its vasodilatory and cellular effects. Being investigated clinically for heart failure, mitochondrial myopathies, and kidney disease.
