What It Is
TB-500 is a synthetic fragment of thymosin beta-4, a natural wound healing protein. It promotes cell migration and differentiation by sequestering actin, which is crucial for cell movement.
Mechanism
TB-500 promotes cell migration and differentiation by sequestering actin. It has anti-fibrotic properties — it down-regulates TGF-β and other scar-forming pathways. In practice, TB-500 improves the "glide plane" of healing tissues, meaning wounds heal with more elasticity and less adhesions. It also prevents adhesion of fibroblasts that lead to scar contractures and encourages proper cell migration.
How Dr. Emer Uses It
Dr. Emer uses TB-500 as the second core component of the Wolverine Stack, given 2–5 mg SC weekly (front-loaded as 2 mg twice weekly for 2 weeks, then weekly). It is used to reduce fibrosis risk after large injuries such as lipo or laser on scar-prone areas. Particularly useful in scar revision cases or scar-dense fields. In the STABILIS IV, 1–2 mg IV is included for anti-fibrotic and pro-angiogenic support post-procedure.

Safety & Considerations
Off-label research peptide. Thymosin β4 is also not FDA-approved for general use, though it is allowed in compounded form. Doses used are considered safe; occasional reports of fatigue or transient edema exist. Caution against use in cancer patients as it might encourage metastatic cell movement. No organ toxicity known. BPC-157 and TB-500 are typically banned by WADA (S0/S2) for competitive athletes.
