Composition
What's In This Stack
Melanocortin receptor agonist that works at the brain (hypothalamus) to trigger libido and arousal. FDA-approved for hypoactive sexual desire disorder in premenopausal women; used off-label for men as well. Can cause increased sexual thoughts, genital arousal, and erectile firmness via central mechanisms. Emer uses it as a "date night injection" for patients who need a boost in desire.
Dose: 1.75 mg SC as needed, ~45+ minutes before sexual activity (no more than 1 dose/24h, 8 doses/month per FDA guidelines)

The "bonding hormone" — intranasally it can enhance sexual climax, emotional connection, and has some pro-erectile, pro-orgasmic effect via sympathetic dampening and smooth muscle relaxation. Peptide that stimulates the hypothalamus to release GnRH, thereby increasing LH/FSH and downstream testosterone or estrogen production. Used primarily in men with borderline-low testosterone who want a boost in fertility or testicular function (as an alternative to hCG) and in some women with HSDD tied to hormonal suppression. Provides an "endocrine nudge when labs support" — naturally coaxing the body's hormones up rather than using exogenous hormones.
Dose: 10–50 mcg SC nightly (in cycles, e.g. 2 weeks on, 1 week off)

For a man with ED and low libido: daily tadalafil continuously, PT-141 injections 1–2×/week as needed, and if low-T, kisspeptin cycles at night (or hCG) to raise testosterone. For a woman with HSDD: PT-141 before anticipated sexual activity, intranasal oxytocin each time, possibly kisspeptin if estradiol is low. Tadalafil daily can be chronic; PT-141 is limited by frequency due to tolerance/nausea if used too often. All come with counseling on lifestyle (sleep, psychological factors) as part of a holistic sexual wellness approach.
